Self Certifying Affidavit (This is the form you need for inspection)
At this time we are still utilizing the above Self Certification Affidavit for all residential inspections. This process requires a representative of the property, either owner, buyer, real estate agent, lawyer etc. to conduct a thorough physical inspection of the property according to the 5 items listed on the affidavit INITIALING (not an X or Check) declaring that each item meets the current fire code according to the information provided. Whomever does the inspection and initials the items also signs as the applicant and has the form notarized then submits it along with payment to GTFD #2 located at:

Please note that payment is determined by the amount of business days prior to-but not including-your settlement/rental date.
Payments and affidavit submissions may be mailed or placed in our after hours drop box located on the far right corner of the fire station by the front door to the Fire Department Offices.
Once processed, executed affidavits and inspection certificates will be emailed to the address identified on the affidavit. Please ensure that all information is printed clearly to avoid a delay in processing. The Fire Official reserves the right to conduct a full inspection of any self certified property at any time.
With COVID restrictions being lifted we anticipate being able to resume in person inspections and applicants will be advised once a date is set.
Effective July 8, 2018 GTFD#2 entered into a Shared Services Agreement with GTFD#4 to staff a fire engine 24 hours a day, 7 days a week. This schedule will greatly benefit our residents with shorter response times for emergency calls and guaranteed staffing around the clock.
* Since the inspections are conducted by our career firefighters, inspections are sometimes preempted by emergency calls for service. A two hour time frame will be given for the crew to arrive at your property to conduct the inspection on your scheduled date.
Vacant premises may be entered by our Inspectors providing a lock box code has been provided at the time of application and there are no personal belongings within the property. Upon satisfactory completion of the inspection, 2 copies of the Inspection Certificate will be left on the kitchen counter. Absence of the inspection certificate indicates that the property has failed the inspection for one or more violations and that you will need to contact the Fire Prevention Office to Coordinate a re-inspection.
When completing the application, please ensure that your printing is clear and legible and indicate with an asterisk * which phone number we should contact for the appointment or should we have any questions.
While we strive to complete all of our appointments in a timely manner, our first priority will always be emergency services protection. All of our inspectors are active, career firefighters responsible for responding to emergency calls-this may cause a delay in the timeliness of your appointment. You will be notified at our earliest possible convenience about rescheduling your inspection should your appointment be missed due to a fire call.
